Box 4500, Allen, TX 75013 Re: — Account ending XXXX To Whom It May Concern: This is a formal dispute submitted under the Fair Credit Reporting Act (FCRA), 15 U.S.C. §1681i(a), regarding inaccurate and/or unverifiable information appearing in my consumer file. Disputed item: — account ending XXXX Reason this item is inaccurate, incomplete, or unverifiable: Requested action: Delete this item in its entirety from my consumer file. Under 15 U.S.C. §1681i(a)(1)(A), you are required to conduct a reasonable reinvestigation and either delete or modify the disputed information within thirty (30) days of receipt of this notice. Under §1681i(a)(2), you must forward all relevant information I have provided to the furnisher within five (5) business days. Under §1681i(a)(5)(A), any information that cannot be verified MUST be promptly deleted — not merely "updated" or "verified" via an automated e-OSCAR ping, which courts have repeatedly held does not satisfy the "reasonable reinvestigation" standard (see Cushman v.
