This is a formal dispute submitted under the Fair Credit Reporting Act (FCRA), 15 U.S.C. §1681i(a), regarding inaccurate and/or unverifiable information appearing in my consumer file.

Under 15 U.S.C. §1681i(a)(1)(A), you are required to conduct a reasonable reinvestigation and either delete or modify the disputed information within thirty (30) days of receipt of this notice. Under §1681i(a)(2), you must forward all relevant information I have provided to the furnisher within five (5) business days. Under §1681i(a)(5)(A), any information that cannot be verified MUST be promptly deleted — not merely "updated" or "verified" via an automated e-OSCAR ping, which courts have repeatedly held does not satisfy the "reasonable reinvestigation" standard (see Cushman v. Trans Union, 115 F.3d 220 (3d Cir. 1997); Johnson v. MBNA, 357 F.3d 426 (4th Cir. 2004)).

I further remind you of your duty under 15 U.S.C. §1681e(b) to follow reasonable procedures to assure maximum possible accuracy of the information in my consumer report. Continued reporting of information you cannot verify exposes you to liability for actual, statutory, and punitive damages, plus attorney's fees and costs under 15 U.S.C. §§1681n and 1681o.

Please send written results of your reinvestigation and a free updated copy of my consumer report to the address above, as required by §1681i(a)(6).

  • Copy of government-issued photo ID

  • Send via USPS Certified Mail with Return Receipt Requested (green card).
  • Mail one personalized copy to EACH bureau reporting the item — do not photocopy a single letter.
  • Keep the certified mail tracking numbers and a scanned copy of every page you mail.
What to expect
